So if Susan is the big bad/one who waits as some have speculated, I sort of have a bit of a fan theory on how that could have happened.

While the Doctor usually regenerates alone or near companions or friends and therefore this ability is not known to the world at large, we know that earlier the Timeless Child and later, Jack Harkness (In Miracle Day) were treated as test subjects/tortured because of their regenerative abilities.

Also at some points in the show, the Doctor's regenerative powers or other aspects of his time lord biology were sought by less sympathetic parties. The Dastari/Androgum/Sontaran alliance in Two Doctors (although this was the part of the brain that had symbiosis with the TARDIS), the mutant aliens in Mawdryn Undead, Davros and the Daleks in Magician's Apprentice/Witches's familiar etc.

What if, at some point, the Carole Ann Ford incarnation of Susan passed on and regenerated, but the humans around her (probably not David who might have passed on by then)-perhaps terrified by aliens in part due to the Dalek invasion-saw this and experimented on her next form (also perhaps while it hadn't adjusted or wasn't near any TARDIS, or other calming environment such as rest to help stabilize it) perhaps driving her mad and feeling abandonment from the Doctor. Maybe she eventually finds some TARDIS or other time travel tech and becomes this "One who waits" entity, following the Doctor.

Also Carole Ann Ford is still around, was seen at the premiere so she can still play Susan post Five Doctors but pre-regeneration.

    Episode 3-

Now that's a bit more like it.....

also a few surprises pretty much right away.

Also the trailer shot from before the specials finally shows up.

Next episode is the one with the TARDIS on a cliff side and the Doctor missing, seems kind of a "Spooky" episode involving a rural Welsh village, a mysterious woman (The Susan Twist character? Mrs. Flood? The Woman who left Ruby? All three?) etc.
