In Civil War II Thanos came to Earth to get a Cosmic Cube and when a giant team of bricks came to knock him up he gave Rhodey a big energy punch that killed him. The trauma he has in the current book is because of that but also earlier trauma he has had in the suit like when he became a cyborg in the 2000's or when the suit caught fire and he was falling to Earth in the 80's. Bad things happen to good boys.

I expect Tony won't be in it. But it's certain he won't be gone for good and this looks like it's going to be a fairly compact series.
I'll enjoy it while it lasts and look forward to Tony's return. And who knows, he might actually play more of a part in this than one thinks.

So a lil birdie told me Tony appeared in Contagion, anyone knows what’s up?

He gives Ben a pep talk ("we just got the Fantastic Four back, we're not losing them now") and tries to obtain biological samples of the contagion to study so that the Avengers can cure the infected (which includes Reed, Sue, Johnny, Doctor Strange, Luke Cage, Cloak and Dagger, plus many civilians). But the last we see of Tony in issue 3, some of the spores are on his helmet... looks like he's about to be infected himself.
