Junior (Alex Merkel)
Dan Didio? JK!
Two excellent suggestions! (Especially Dan Didio)
I'd add Murmur to the list. Doctor is one messed up bloke....
The Batman Who Laughs is horrifyingly overused.
Professor Pyg for me
Zymyr or the Gil'Dishpan race.
All they do is float around and telepathically torture people

Definitely The Joker:

Scarecrow, Brainiac, Grodd, or Darkseid.
Batman.
No, not Batman Who Laughs, or some other Dark Multiverse version, not Owlaman, or even Fatman. Just Batman.
Lex Luthor. Cause he is the kind of villain that can become president.

Probably, or The Scarecrow:

Nekron - he's the embodiment of death.
Joker, Scarecrow, Dr Psycho and the Upside Down Man.
Batman.

The Batman Who Laughs just tries too hard to be the most horrifying villain in my opinion.
