1. Morrison/McKean - Arkham Asylum - A Serious House On Serious Earth
2. Azzarello/Bermejo - Joker
3. Chaykin/Moore/Chiarello - Batman/Houdini - The Devil's Workshop
I think it's Brian Azzarello.
Could the guy on the upper right corner wearing what looks like a smoking jacket be Bob Kane? The woman next to him is certainly terrified, but then again, his head...
I see a lot of famous people, but only a couple comic-related.
Silver Surfer's hand is pointing to Dude from Big Lebowski. In front of him is Billy Corgan wearing white t-shirt.
Behind the guy...

For some reason people often forget Spawn/Batman (1994) when talking about Miller's Batman.
Not that I know of.
Yeah, that's dumb. But from business point of view it's reasonable, to get people to buy collected editions etc.

You may want to check Catwoman: When in Rome by Loeb / Sale also.
Nope, though Grant Morrison may disagree.
Heh thanks, guess I have a thing for darker stories with limited Batfamily. I think the books you mentioned are often overlooked/forgotten when people list their favorites. Or maybe they just don't...
