I can't remember the name of the movie but it's got a lumber jack tryp cutting a trees and the tree falls on the guys legs and then water starts to flood the area the chainsaw gets water damaged and they can't free him he goes under water they take turns going under water giving him air but he drowns really scary.
The Hot Tub scene in About Schmidt is pretty horrifying. But on a serious note, the Glass in the feet in the original Die Hard. I just took a small, tiny piece of glass out of my foot a few days ago and that thing hurt can't imagine dozens of large shards cutting into my feet.
The hospital scene in Spider-Man 2 when Doc Ock’s tentacles went crazy.
Scared the hell outta me.

Because the scene in Robocop where Murphy gets shot up haunted a generation of kids. Myself included.
There's an Agatha Christie adaptation where you realize the murders have been the work of a precocious child that's pretty chilling.
Personally, I found the knife-fight in Saving Private Ryan among the most horrifying things put to screen.
There's a scene from a Tyrone Power film where a Borga-esque character gouges out the eyes of a helpless captive. Turns out to be a fake, but it's hard to watch.
Hopefully this counts, but my choice would be The Boar God Rampage from Princess Mononoke (1997):

That part where Charlie from Lost got a high profile job offer (before losing it spectacularly due to drug abuse effects) spooked me.
Indiana Jones of course has it's share of horrifying villain deaths (Not just the supernatural ones, but Pat Roach's characters get sliced and crushed to death) although they deserved it.
Although the guy who has his heart cut out and lowered into the pit probably didn't.
The various corpse/skeleton jump scares in the films are pretty freaky as well (Forrestal in particular as his head *turns* in Indy's direction!), although they seem to be mostly in the first two films.
Indy's transformation into evil Indy from Temple of Doom is pretty freaky too.

There's some genuinely unsettling scenes in James Bond.
Cruel and unusual deaths of certain Bond girls Jill (Goldfinger-covered in gold) Helga (You Only Live Twice-Eaten by Pirahnas) Corrine (Moonraker-Mauled/ possibly eaten by dogs) Solange (Casino Royale-Tortured and wrapped in a hammock, makes Tobias Menzes's character visibly sick) Strawberry fields (Quantum of Solace-Covered in oil-Goldfinger homage)

Although the villain is largely played for camp in his two Bond films there are some unsettling bits with Jaws.
SPY WHO LOVED ME
His debut kill near the Pyramids, and his Max Kalba kill in the next scene. Both cut in an interesting way with the festivities (Pyramid light show and music) going on just outside.
Hiding in the closet on the train. Even scared Richard Kiel.
MOONRAKER
Jaw's attack in the alleyway in Brazil in the costume. When a bunch of Carnival-goers pass by and Jaws just tries to pass them off as being some fun couple but she's absolutely terrified!

Thankfully Bond shows up and saves her.
LICENSE TO KILL also has the quite terrifying Felix maiming.
MOONRAKER also presumably has all of Drax's "Ark" dying. Although, on the other hand, they are seen in various parts of the film being complicit in Drax's crimes and watch Bond's near death by python.
Also the decompression death of Krest.
