No requirements. Usually, it is a five week month and a book that Marvel thinks people will spend money on a second issue.

Time was, pretty much every ongoing title would get an Annual issue. Nowadays, with all the short runs and relaunches, I think it's like Mike_Murdock says, and it's anyone's guess.
Wasn't Spidey's latest Annual legacy Numbered? That would be a nice touch to collect the annual issues as well.
Well, X-Men Blue's annual is actually the first part of a crossover. X-Men Gold's is a one-shot story that could potentially backdoor pilot an Excalibur series at the next relaunch.
