    I don't think one was. There's nothing to stop anyone from voting for it twice either. What's worse is when you use a cover and no one votes for it. Then it gets used by someone else later on and wins

    I don't know if a cover's ever won more than one contest but I wouldn't be surprised if any have, especially a beauty like this one. & No, I haven't heard of any rule about a cover being 'retired' once it's won, but that idea never even occurred to me, I think any comic cover's fair game.
