I'm just saying that despite his rep, Joker pretty much does the same stuff as dozens of other villains. He tortures, he murders in gruesome fashion, he kills people the hero cares about.

Sure he does, but he does so that leaves an impression that very few (if any) other supervillain leaves. That's where the psychological aspect comes in and what ranks him one of comic's greatest supervillains.
