Funny thing is there are two death head 1's running around. (and that does not count death head 2 and 3!)

True, but doesn't the S.W.OR.D. comic pre-date both of those in terms of the character's chronology? Truthfully it probably doesn't matter; whatever universe The Body In Question takes place in is the issue so wherever he comes from in that is ultimately the answer. I have no idea what that is though.

TRN means there's no official reality name. Temporary Reality Number is a system the wiki uses to disambiguate such cases. If an official designation is assigned, the TRN gets retired.

Thanks - I didn't know that! In any case, I think with Death's Head it may be a fruitless task trying to say where he came from. There is always a further option, and Simon Furman seems happy to leave it speculative, so I doubt it will ever be nailed down.
