For some reason some characters are most commonly referred to by their actual name as opposed to their Superhero name. Examples are like Luke Cage (as opposed to Power Man) and Donna Troy instead of Wonder Girl.

Jean Grey, Emma Frost, Kitty Pryde ...
Monica Rambeau, mostly cause her superhero name has changed so often no one can keep up.
It's normal for fans to do this for pretty much all heroes (Tony, Steve, Hank, Wanda, Janet, ad nauseum).
It's also normal for the "family" (fellow comrades) to use real names casually.
It starts becoming a problem when other characters (outside the group) do it. Iron Man and Cap better not call her Kitty.
Jean Grey: Started because "Marvel Girl" is just such a horribly lame codename that should stay in the 60s, and Rachel was Phoenix at the time. Even the 90s cartoon announced her as just "Jean Grey."
Emma Frost: White Queen was her title in the Club, and when she left, there's no real reason for her to use it... or dress in white, for that matter. And everybody was coming out of the closet at that time. If she was going to be diamond, I've suggested Ice Queen.
Kitty: Just don't know why more don't love Shadowcat.

Reed Richards is kind of in-between - he's still technically "Mr. Fantastic" but not a lot of people seem to call him that, except to make fun of the choice of name.
Hmm.. Nick Fury, Elektra, John Lynch, Namor, most of the gods (Thor, Hercules, etc.), nearly all of the Guardians of the Galaxy (Groot, Gamora, Vance Astro, Rocket Raccoon, etc.), Rupert Thorne, Harley Quinn, Ra's al Ghul and Talia, Quentin Quire, Sebastian Shaw (and most of the Hellfire Club members), Siena Blaze, Shang-Chi, Cassandra Nova..

Thirding Reed Richards, as well as really all of the core FF. At least the FF faithful refer to the FF by the given names a lot more than their code ones (although with Ben and Johnny, it's 50/50 between their being referenced as Ben and Johnny and Thing and Torch) anyway.

I think the test is what a villain would say when he has captured a hero. He might say "I have captured the Invisible Woman!!" or "At last, I will do away with the accursed Thing!!!" But I can't see many of them referring to Reed Richards as anything but Reed Richards.
The same goes for a lot of the mutants: yes, they call each other by their real names, but bad guys say "Bring me the Scarlet Witch and Storm," not "Bring me Wanda and Ororo." But a bad guy would have to call Jean Grey "Jean Grey" because no other name has stuck.
I think Doom and Dracula are the only villains I can think of that call's the Hero by their name rather than code names. I guess they find it childish or just stupid that an adult will go by a code name.
For a counterpoint example, Steve Rogers is so universally regarded as Captain America that even in universe close friends and virtually all fans refer to him as Cap rather than Steve.
