if they really wanted to shock the audience what should have happened in the elseworld was iris kissing kara and calling her Barrie Honey. they would also have been step siblings naturally.
Alex would be Barry's step sibling( not a lesbian) and Barry would consider her his love interest, brooding over her secretly while he flies in the skies. [not meaning it seriously but still...]
