The sheer scale of Q’s reality manipulation is way in excess of anything Genie has demonstrated.
Good lord. We're talking a genie that's a 5th dimensional imp vs. well... Q which is basically a 5th dimensional imp.
Genie never unmade the Universe or walked off the Big Bang.

Q wins this. His powers are limitless. Even the afterlife is a playground to him.
As powerful as Q is, he has no feats whatsoever when it comes to musical improvisation at all. Sure, he can summon a mariachi band, but nothing that displays his own skills.

Regardless, it's scale. They both can do almost anything but Q can do those things on a scale that the Genie cannot. Well, who knows? But that's the No-limits fallacy. We have no evidence that the Genie can do things on the scale of moving planets, time travel, teleporting galactic distances, surviving forces equivalent to a star exploding, etc.
Power with Girl is better.
Genie did some bonkers stuff in the Aladdin animated TV show. He was explicitly weakened after being wished free. Despite this he was still doing things like ice skating on the rings of Saturn, mucking about with Godzilla sized Kaiju and generally messing with the laws of nature. Pulling people into mirrors, showing things happening a great distance away, breaking spells, shape shifting and so forth.
None of that sadly much compares to a Q's higher end stuff.
You would need to take his and Jafar's feats and statements super literal and at face value to start matching up. He states he has phenomenal cosmic power while manipulating stars, planets and space - later recreated by Jafar. Jafar also said things like "all powerful" and "the universe is mine to control! to command!". So. Only problem is, the stars and planets and such were obviously not full sized, so it's very easy to argue it was more just poetic wording and an impressive light show as opposed to a real feat of macro scale reality warping.

Ah, but that's just playing an instrument. Genie made up lyrics, choreography, and special effects on the spot. Q is not so poetic.
That reminds me, for all of Genie complaining about vast cosmic power, itty bitty living space, a Q (and all Q have the same powers) once shrank Voyager and transported it to the Big Bang. Every proton was roughly the size of the ship itself, but it also meant essentially an onslaught of same-size projectiles flying all around her and just the proximity of those protons flying past was was rocking the ship.

...Why does this remind me of the Aladdin sequel in which Genie threw everything at Jafar but he gets his ass whooped?
...Then again, he was part of a villain song so he was definitely screwed there.
