When I went to see the Tom Cruise War of the Worlds, some people brought a baby into the theater. The kid started crying and instead of taking the baby out of the theater the Mother and Grandmother went to the back seats right behind me.
Not quite as bad as this. But a girl at work wanted to see The 5th Element and had me take her. About half way into the movie a couple guys sat in front of us reeking of cigarette smoke.
Like they hadn't changed clothes in a month. And the theater was too crowded to try to find other seats.
I went with my brother to see a movie. I went to get seats, and he went off to get snacks. He was clearly annoyed from the moment we arrived, and as soon as he enters the theater itself, he screams out at the top of his lungs "KEVIN, WHERE THE **** ARE YOU?!"
I was later confronted by a father, who really let me have it (just verbally). "I brought my kids to this!"
That... was mortifying.
Spider-Man: Homecoming. During the ferry scene lights started flashing at the exits, the lights came on and I thought, with 100% certainty, my friend and I were going to have to make a break for it before someone started shooting. Someone pulled the fire alarm and we had to wait outside for a half hour while FDNY secured the building.
When I was kid while seeing Licences to Kill a bad storm took the power out half thru the movie emergency generators kept lights on but we had to wait half hour for the film to start again my old man and sis who was in her twenties was happy they went into the hallway to smoke the 80's.
Theater overbook Ghostbusters 2 and people were seating in isles it was nuts.
Almost walked out of Batman and Robin nuff said.
Sister kept asking me questions during Star Trek First Contact.
A lot of talking and cell phone usage during LOTR the first film.
ATOC realizing I was cringing when others were cheering felt like I lost my fandom that day went from Avid Star Wars fan to Casual.
Crying baby during X2.
That's about it.

Kingpin- saw it with a friend and his friends as part of his birthday party, but I remember we spent most of the movie having an ice war with a couple of jerks a few rows behind us.
deathly Hallows part 2- It was a 3-D IMAX show. About 2 hours before the show, I started to feel sick and decided not to go. About 30 minutes before showtime, I remembered how much the ticket cost and got my ass to the theater. Had to sit in the first row near the end, and spent the whole movie craning my neck and looking left to right like I was at a tennis match so I could see the whole picture.
Almost forgot had the one of the worst Heart burns I had in my life while watching Thor the Dark world. No they werent related but that movie did nothing to take my mind off of it.
I saw Avatar in 3D with a collapsed lung. I thought it was just chest pains. Turns out I had to be emergency escorted from work the next day. Made watching the movie a bit difficult, too.
My 1st less than stellar experience was an IRA bomb threat in the shopping centre where the cinema was. Stopped the film in the final third. I still haven't seen the end of Lawnmower Man.
2nd was the Peter Jackson Kong, lovely smell of vomit across the aisle and crying small child (why it was there I have no idea).
3rd was right at the front, extreme left seating for Blair Witch Project - being that close to a shaky-cam film really made me feel queasy.
I cannot point directly to one instance, but I am 100% sure, it involved other people talking non stop on the phone. Think there was even an argument between some of the audience.
Otherwise no major malfunctions or evacuations that I can think of.
EDIT: Yes I remember also watching a raunchy comedy in a theater full of hormone crazed teens. I was in my thirties at that time.

I saw one movie where some kid in the audience kept shining a laser pointer at the screen.
Finally a deep voice said "You better put that laser pointer in your pocket or you're gonna lose it."
That was the last we saw of the laser pointer.
