Hulk killed that dog from his Joe Fixit days, the guy who gave him Gamma Cancer (Madcap?) and that douche scientist from Immortal. They also retconned him having causalities from his Las Vegas rampage from before Planet Hulk.

Not sure about Cain though.

lol you don't know what you are talking about. madcap doesn't have anything to do with the hulk and AFIK the hulk never has suffered from cancer but has a healing factor that mimics cancer like growth of cells. the dirty rat that did get cancer was Rick Jones not Bruce.

Hulk has been in some high stakes situations that regular people stand no chance of surviving.
