All the others you mentioned, plus many more, were on the timeline Mark Waid worked from, but there's only so many pages, so I guess he felt the other events listed were more relevant to the overall tale. However, Devil and Moon Boy are not from Earth-616's prehistory. They originate from the modern day alternate Earth, as evidenced in Fallen Angels and other stories.
The Hyborian Age did take place on Earth-616.

What we do have is incredibly wide-ranging and covers the most significant events nicely. But yes, with the best will in the world, there was no way he could cover everything without massively upping the issue count for the series.
