Pretty simple: It's Yonkou vs. Yonkou.
The Adopted Father vs. the Awful Mother.
Who takes the win?

Big Mom just doesn't have the kinds of feats Whitebeard has. Kaido might turn out to be a good match, but Big Mom isn't.
She's massively strong and durable, no doubt, but Whitebeard is plainly stronger than her (stopping a charging ship one-handed when already mostly dead, creating massive gales enough to fill the sky with marines just with one normal swing) and his Quake Quake fruit is perfect for wrecking her durability.
Big Mom does have a bit of a versetility edge thanks to Zeus and Prometheus.
But Whitebeard beats her out in raw strength.
He should win, but it'll probably hurt afterwards.
Zeus and Prometheus will put the hurt on, but it's nothing Whitebeard hasn't amply shown he's capable of fighting through. Both of them are massive tanks with great offensive power, but Whitebeard is both physically stronger, more skilled (Big Mom is just a brute) and has the more devastating range game.
Furthermore, Big Mom's insane durability is balanced out by the fact that her damage soak once that durability is bypassed isn't that great. Simply falling to her knees when her durability was down caused her knees to bleed, and having a brachiosaurus divebombed into her head was enough to knock her out. Now, there's no shame in getting knocked out by a massive dinosaur to the head, but Whitebeard literally was stabbed straight through the chest by a giant sword, shot hundreds of times including taking multiple bazookas to the face, was shot several times by Kizaru's lasers, got his torso pumped full of lava, got almost half his head blown off by more lava, and after all that still split a fortress island in half and beat down a fresh Blackbeard mano-a-mano. He will eat anything Big Mom can dish out and hit back harder.
But yes, he'll definitely be feeling it afterwards. Big Mom's standard punches are equivalent to Luffy's Gear 4 punches. Just one Gear 4 punch was enough to nearly take out Donflamingo right then and there, sending him flying across his kingdom to crater into a mountain. Big Mom's punches will hurt.

Not to mention Whitebeard's probably not dying from illness in this fight.
Critics who treat adult as a term of approval, instead of as a merely descriptive term, cannot be adult themselves. To be concerned about being grown up, to admire the grown up because it is grown up, to blush at the suspicion of being childish; these things are the marks of childhood and adolescence.

contrary to what the anime showed, whitebeard had half of his head melted clean off and it didn't bother him at all.
Agreed. Hence my distinction between Big Mom's durability and Whitebeard's damage soak. Big Mom is extremely difficult to hurt in any capacity, but once you get through it she's got a glass chin for her Yonkou weight class. Whitebeard in comparison is very easy to hurt if you can actually hit him, but he can take absurd levels of punishment that hurts him or even destroys vital organs and just keep on trucking.
this is what i mean when i said it didn't bother him.
akainu melts off a good chunk of his head. whitebeard doesn't flinch, stagger or even seem to register that half his face got burned clean off. he just counter-attacks and puts akainu out of the fight for a good while.
I know the scene and Whitebeard's soak is tremendous as others have noted.
My quibble was with your wording. It didn't not bother him, it and bunch of other wounds, literally killed him. They did bother him.
To say that they didn't bother him is to imply that there was no damage or that there were no repercussions to attack which isn't correct.
Basically, every attack that hit Whitebeard bothered him. He was over 70 years old, had some form of heart disease and fought the entire battle with a large stab wound in his chest (thanks Squard), of course it bothered him.
He just didn't let any of that stop him from wrecking shit.
Ok, so we're generally in agreement that Whitebeard wins this with effort. How do y'all think their crews stack up to one another?
Katakuri vs Marco would be an epic, drawn out fight.
