Have any portals been done to/from FTL sources before? The Ship obviously was going that fast.

It wasn't at all going that fast when they were about to crash.

Plus, the surface of the Earth is always moving massively quickly, around it's axis, around the sun AND through the ever-expanding void-ish of space. So those portals don't have any problem at all of dealing with a moving target.

So rather than portal himself to Earth, Strange was asking Stark whether he could fly Maw's ship because...?

Wasn't he just recently tortured? He might have needed his subsequent meditation session to get his concentration back in order, or at least a little bit of time to clear his head.
