I don't know, I think Evil Batman actually has a solid look going on. Very Judge Death/Pinhead feel to it and that's fun.

His posse though... not very good variant designs because they don't commit to the whole black metal aesthetic of it.

More leather, chains and spikes required. Also, they should all have their eyes covered. Dehumanises them, makes them thematically inkeeping with Batman's look, draws focus to the presumably laughing mouth, allows for dramatic reveals of their tears as their flayed human soul cries out for release from this curse that is piloting their body around doing unspeakable horror.

You know, evil possessed versions of heroes isn't a bad concept, you just gotta commit harder.
