I think I would be very afraid of the cannibalistic Marvel Zombies. They were practically unstoppable. Mauvais is pretty horrible in that regard too but he's not as powerful as a Zombie Hulk or something like that.

Beyonder, Dark Beast, Mr Sinister, Carnage, Dweller in the darkness, Dormammu, Chthon, Sabertooth (when he is being depicted as a sadistic serial killer) and the future nihilistic devil Hulk in the current Immortal Hulk book is also pretty scary.
Mephisto is a solid choice. Especially the Buscema version.
Emplate or Dirt Nap
they are both visually and, by their actions, horrific. Nightmare just wants to scare you. Emplate sees others as a food source, can remain invisible until he wants to grab you, and will hide you away someplace others can't reach while he feeds on you.
and Dirtnap literally consumes people, takes their form, and slowly digests them. both prey on children.
