I hope WPP comes in to clarify. I would think that he meant appearances in somebody else's book. Not in anthology books like Showcase or Tales of.. which were really the heroes own book.
Also as mentioned, what about first cover appearance, but second issue. Like Hulk 181, Conan 24 or FF #49? Should the contest cover be Hulk 180, Conan 23 or FF 48?

Hmm! Didn't think any clarification was needed, though I should've been more specific. Okay, cover appearances in someone else's book, no anthologies or trades, as for "Tales Of" titles, well, there had been debuts there, so those can be accepted. I hope this straightens up matters for everyone.
