If they are twins, then they never went to Jusenkyo since that wouldn't make much sense plot wise (not that sense was ever an issue).
Assuming they never went to Jusenkyo, then neither did Ryoga. Not sure about Mousse or Shampoo.
Now, since Ranma went to an all-boys school for a few years, Ranmako may have gone to an all girls school which could have created a female adversary fro Ranmako right off the bat.
Also, Ukio might have had a different relationship with Ranma if Ranmako was there as well. Hard to say. Possible that Ukio might have become close friends with Ranmako, and not been so angry with Ranma.
Later on, of course, Ranma would be dealing with the females in the series, Akane, Shampoo, and so on. Ranmako would be fending off the attentions of the males, Kuno, possibly even Ryoga, and others.
It wouldn't be quite as funny, and not quite as convoluted as an M.C. Escher drawing. There would still be plenty of Martial Arts. Trouble that Ranma gets himself into, he might not need as much help from others if his sister is of the mind ot give him a hand, and visa versa.
A lot of it depends on the personality of Ranmako (aka Ranma chan). We can assume she is as skilled in fighting as Ranma is. Does she like the attention? Is she more interested in training and fighting? Does she make friends easily? Will she be there for Ranma, or will she be a cause of his problems?

Well if they are twins, that gets rid of most of the gender-swapping comedy, as well as a lot of the "it's pervy because a guy is doing it in a girl's body" humor. There could still be some of the "twins switch so it's funny because the worng-gendered person showed up for an explicitly gendered thing" like Male Ranma showing up to discourage guys from dating female Ranma, but for the most part that dynamic will play off as more "brother-sister" than "same-self" it was in the original comic.
That said, without the gender-swapping triggering a lot of the sexism in Ranma 1/2 characters, you may be condemning both of the Ranma twins to an early grave.
Think about it. Canonically Shampoo wanted to KILL Girl Ranma to satisfy her battle-pride or whatever. She also wanted to marry Boy Ranma because he beat her in a fight. She only quit trying to murder Girl Ranma because she found out they were one and the same; and she decided her hormones trumped her battle-pride. So if they're twins, then rather then stopping to try to bone male ranma, she can keep trying to kill girl ranma until she succeeds. And while she was a bit weaker she was WAY better at stealth than Ranma was paranoid.
Likewise, Male Ranma was willing to fight a lot of the other dudes to the death, and only the transformation into a girl (which the other guys wouldn't attack and didn't associate with male Ranma) saved him in some of those fights.
