Definitely Harvey Dent (A.K.A. Two-Face):
Jason Todd. He's more of an anti-hero now but he's clearly not a Robin-level hero like he was at first.
Superboy Prime was originally a hero. He's been purely villainous ever since IC.
Hal didn't stay evil but his turn is a rare example I will always find fascinating.
Lady Shiva and Bronze Tiger were Richard Dragon supporting characters before they became villains.
monarch - former hero turned supervillain
maxwell lord - rich business man with a minor power gets turned into a megalomania mastermind
Triumph - was at the beginning for the formation of the justice league , got timelost and forgotten , by the time he returned , world has pass him by and he eventually turned bad guy
maxima - former supervillain turned jla member turned bad again
brainwave jr - son of a supervillain , former infinity inc member turned evil but good again - not sure
jericho - son of deathstroke , eventually his soul got corrupted and deathstroke had to put him down
Thinking of how that character has been ruined makes me so sad. I mean, the original possession and death sucked, but after they brought him back it was worse.jericho - son of deathstroke , eventually his soul got corrupted and deathstroke had to put him down
