I can see THe Blob being a country singer being that he is from Texas. But I have never seen any evidence that he is a racist or Anti Muslim or Homophobic in any appearance I have read him in.

I honestly don't see that changing much. Julia Carpenter would've been called Arachne due to the name Spider-Woman being taken (Jess was resurrected but depowered by the time Julia debuted, right?) and that's about it.

If Cyclops had actually killed Captain America, that would have been the death knell for any hopes of peaceable human-mutant coexistence. The most visible champion of mutants slaying the Sentinel of Liberty, even under the influence of Phoenix Force-driven psychosis, would be played over and over in the media and the government would likely leverage that as a means to get the superhero community onboard with the enforcement of whatever anti-mutant measures it could drum up.

Funny enough, there was a What If? that came close to the second, with Scarlet Witch taking away everybody's powers, mutant or not, and as for the third, assuming that the newly superpowered human race didn't completely collapse on itself in an orgy of superpowered chaos, I could see the situation turning out like My Hero Academia, where it would be those without superpowers treated as an anomaly at best.

Doctor Octopus in charge of the Thunderbolts would have been interesting, too, especially since he was also part of a Masters (of Evil) lineup at one point, and if he managed to execute his plan to restore himself to health using Spider-Man's body/DNA . . . we could get Superior Octopus out of the deal. Also, if Nick Fury still led S.H.I.E.L.D. during Civil War, most of the utter crap that went down wouldn't have happened, because the whole point of ousting Fury back in Secret War was to replace him with someone who'd toe the party line on restricting superhero activity, thus practically chomping at the bit to put superheroes that wouldn't comply with the government in prison.

In the spirit of keeping this going, though, I'll contribute one of my own . . . "What if Spider-Man Never Left Iron Man in Civil War?"

Wasn't Earth destroyed in Uncanny Avengers, and a time rollback needed to fix it? Had Cap been killed by Scott, the Unity Squad would've never existed, and any threats that team defeated would've not been. So basically, it means the end of the world.
