I'm curious as to who hasn't. Polaris is the only one I know for sure has never died in the 616. I'd assume Wolverine and X-23 never have based on their power set but I know I could be very wrong about that.

Wolverine died, it was a big storyline called death of wolverine.
I guess Monet never died, the 3 cuckos that didn't died...

Monet died in House of X but other than that I think she had a few death moments in earlier books ( either "died" or near death). Someone correct me but I think maybe in X-factor (X-factor 255 maybe?) and maybe a story in the all female X-men book (again I'm not sure about this one .. I could totally be wrong).
Beast and Iceman are the most senior X-men that never died.
Of the ANAD X-Men, Sunfire got his legs chopped off and kidnapped by apocalypse but don’t think he ever died.
Thanks, X-Factor #120 is one of those cases where I don't know much about it and kinda just shrug over that period. I skimmed it and I noticed she's using a gun for some reason, so on a personal level I'm gonna treat it as a time when Lorna's powers weren't working properly. It does fit the original question of the thread though.

Beast and Iceman were both victims of Thanos's snap in Infinity gauntlet. So were Archangel, Jean Grey, Firestar, Quicksilver and Northstar.
This was when the original line-up of X-men were still X-factor. Also, Captain America tried contacting the X-men and Excalibur but there was no response so we don't know what happened with them, except for Wolverine who Dr. Strange was able to find and recruit.
