Debatable if any Spielberg film qualifies as a hidden gem/underrated but Duel , his first theatrical film, is a favorite of mine. A very good highway horror/thriller movie that's essentially a proto-Jaws but with a truck.

I forgot to add my cult favorite - Knight Riders. Cheesy but fun Ed Harris film with a guest appearance from Stephen King

Sean Penn, Gary Oldman, Robin Wright, Ed Harris, John Turturro - powerhouse lineup from "State of Grace" a great undercover cop drama.

Matt Damon, Scott Glenn and Lou Diamond Philips also give really good supporting performances.
I saw that in theaters. I don't think many did if I recall. I think it was even panned. I still like it though. The performances were very good,

They played Duel on TV a lot. Late at night, though. Its a great movie.

    Thanks for the responses guys.

    A couple of things...

    - Ed smacking that guy around the diner at the end? It is great each and every time.
    - Tom Savini playing Sir Morgan never really gets the credit that it should as a performance.

Oh, I'll add Repo the Genetic Opera (I'm aware it's gained a cult following of its owns too).

You first have to be a fan of musicals, and even in that category, its really violent and dark. It's visually impressive though, Anthony Stewart Head and Alexa Vega in it as father and daughter.

Also wanna add Black Hole and Renaissance. Renaissance is like if Sin City was animated. Truly brilliant.

Predestination and Moon were two really good, mostly under-the-radar sci-fi films.

Also forgot to mention Sunshine, another great sci-fi film with a surprisingly strong cast.

The Comic (1969) starring Dick Van Dyke, directed by Carl Reiner. A silent film comedian is undone by his ego and selfishness. The films within the film are hilarious!
