    I think Vixen was living in Manhattan at one point, but I don't remember which era that was.

    The Hawks were traditionally set in Midway City. Though after Johns brought them back, he had Hawkman operate in St Roch.

    - Catwoman is typiacally in Gotham
    - Red Hood and the Outlaws were also in Gotham from time to time
    - Red Hood/Arsenal were in LA
    - Anarky was in Washington for a while (after No Man's Land) and is appart from that usually in Gotham

    And there are some cities missing from your list: like for example Coast City, Hub City, Ivy Town, Midway City, Opal City, Star City ...
