I have a huge list I've already posted several times in the Classics & Collected forum's threads on the same topic. So to avoid sounding like a broken record, I'll just say DC's biggest sin is starting so many lines (mostly 80's and 90's series) and then abandoning them 2-3 volumes in. It's maddening!

It depresses me how huge chunks of DC's Golden, Silver, and Bronze Age remain obscure and uncollected.
I know DC justifiably has no interest in reprinting comics that don't feature their most popular characters, but they should consider licensing out the rights to reprint their more obscure properties like the Fawcett characters, the Charlton characters, and the Quality characters to other companies, considering they would never get around to reprinted them anyway.
The Levitz era Legion of Superheroes (the Baxter series). Over 60 issues of prime Levitz and only the first 13 issues have ever been collected, and those are long out of print. Such great stories as Who is Sensor Girl, The Death of Superboy, The Universo Project, Conspiracy, and others never collected in any form.
