I am currently reading the Michael Zeck.Artist Edition, original art, printed full size from scans. He is known for his work on Punisher (among others)
Here is one of his cool covers, in color and black&white.
There came a time when the Old Gods died! The Brave died with the Cunning! The Noble perished locked in battle with unleashed Evil! It was the last day for them! An ancient era was passing in fiery holocaust!
Gah! Conn Seanery beat me to it! So, I'm going to go with issue 3 from the same series. Marvel has actually homaged it a couple of times

There is an interview with Don Pendeleton in Marvel Preview #2, which also contains the first Punisher origin story. I'm sure the topic comes up, but it's been years (probably decades) since I read it so I don't remember any details. Of course, this was also when the Punisher was a comparitively new character and years before he became huge.
Woulda believe the Punisher undercover in a frog suit? Well then, let's go with this.
