I genuinely like how this cover has Barry changing from his death from Crisis on Infinite Earths to his return in Infinite Crisis.

I assume that's referencing Wally and Bart shoving Superboy Prime into the Speed Force, not Barry. Wally was being a little goofy because he poofed away to drag his wife and kids with him because Didio wanted all of them dead.

I just assumed it was a play on the idea of Barry popping out to help Bart drag in Prime. The first image looks like the decaying Barry from COIE.

Interesting. The first one was a reference to Flashpoint/new 52 but this is a reference to Infinite Crisis, . . .

Are they maybe skipping over Final Crisis?

Maybe. Or maybe they are not doing it in chronological order. But if it had to do with Flash as a central theme in a crisis, well, Flash has history.
Maybe the last one will be Dark Crisis. Or maybe Metal will be added... but there Flash had not a relevant part at all.

Barry came out of the Speed Force and grabbed Prime (along with Johnny and Max), Wally and Bart were the ones pushing him in.

That's right. The cover shows only Wally, Jay and Bart.

The part where Wally's being all wonky and stretching back and forth is literally right out of that scene in Infinite Crisis. COIE has Barry wasting away, not being all Cartoony.
