Anyone want to hazard a guess as to when "Al Ghul" became her last name because it wasn't until relatively recently. "Al Ghul" was her father's title, not surname.

Wasn't aware she ever had a different surname to be honest.

I know Ra's Al Ghul is a title but I assumed that his family adopted the name just like ra's himself centuries ago.

She was just Talia, no surname for a long time. And when she was working in Lexcorp to sabotage Lex Luthor from within his own company she went by Talia Head.
