Wouldn't it have to be the comic that started the whole "Women in Refrigerators" trope in comics that was coined by Gail Simone, Green Lantern #54 vol 3?

That one really even much of a "Surprise..." though?

At that point in the story, we've got the math on his grandmother's plan for him and the fact that he was a preacher when the title started.

Knowing all of that, there would really only be two possibilities(well, maybe three...)

    - He went back willingly.
    - Jody/TC found him the same way that they found his mother.
    - Maybe... He had just never left?

All that said, it wasn't much of a surprise to me when I was reading it. Matter of fact, it felt like the most sensible way that it could have happened.

Hopefully this counts, but my choice would be the James Howlett revelation from...

When you let the heroes know, there ain't gonna be a happy ending this time, and a genocide has already taken place.

That time Wolverine had to kill a kid because his mutant power was an uncontrollable death field.

That one was a pretty creepy...I certainly wasn't expecting the comic to start off with that scene Did Geoff Johns leave Marvel after that? Here's an interesting article about the comic : Avengers #71 Delayed, Returnable

It actually has a second questionable scene with spoilers:
Whirlwind beating up a prostitute dressed as the Wasp.
    end of spoilers

For me it was the first time I read The Night Gwen Stacy Died. Amazing Spiderman 121 and 122. I did get to read them when they first came out due to you know not being born yet, So Wehn I read the back issues I was floored about the fact that a super heroes girl friend was killed.

When Spider-Man killed Charlie with one punch, that was shocking to me. Not because I blamed him for it but because it was done in the first place.

Hopefully this counts, but another favorite would be the 1st full appearance of...
