Because of the prep, I give this to the Red Skull. The Joker sometimes does prep, but his natural tendency is to improvise on the fly. When the Red Skull preps, he does stuff like conquering Latveria, or squeezing out a Cosmic Cube, or taking a Cabinet-level position with the U.S. government. Come to think of it, the interaction between Red Skull and Cap is a mirror image of the interaction between Batman and Joker.
Joker is good at killing a lot of people without getting caught, but Red Skull tends to go bigger and with more weapons.

And the movie versions reflect this.
Heath Ledger Joker blows up hospitals and ferry boats; Hugo Weaving Red Skull gets ahold of the "tesseract" (an infinity stone), and tries to vaporize capital cities.
Even if this were a no-prep arena match, many versions of the Skull have him jacked on super-soldier serum.
The best outcome is for Red Skull to kill Joker as Bats is too chicken poopy to do that. As far as the fight, who knows. If we discard super duper tech, Skull has the advantage.
Didn't Joker out maneuver Lex Luthor and the Legion of Doom, pretty much having them all at his mercy just to prove the point that Batman Who Laughs was even worse?
I still think Skull takes this, but Joker's no real slouch when it comes to prep either.
