Re: Scenario 1- If the Doctor isn't high that day it might be a short battle for the Vitrumites as the Doctor turns their internal organs into cottage cheese or something. I think the team can keep them off him until he pulls that off. He could also panic, or freeze up, so there's that too. So, 50% chance, either way.

Not familiar with the other incarnation of the Authority.

A competent doctor is basically blitz proof, but he's generally not competent, so... I don't think anyone else can particularly slow the Viltrulmites down from blitzing.
