Ok so that means Thor was the only duplicate then right ?

No, Hulk was a duplicate as well. Sort of. Banner was separated from Hulk prior to his entering Onslaught yet still became Hulk after being reborn.

Ouch! When Ian Zola started using the name, I just figured they'd left her in limbo somewhere. I didn't realized Marvel killed her off.

Yeah she died or better said she was retconed to dead. They made it so that Rick Jones died in the negative zone and that Rick Jones who made it to the 616 was just a part from Onslaught essence. It was a part of a plan to pull himself out there.
