There is already a Spiderman in Ultimate The End. He appeared in the book months before Secret Wars 4 when the life raft was opened. I am certain Thanos was sent to the other side of t be Shield. Most likely to keep him out of the Illuminati's way so they can deal with Doom. On a future cover Thanos is getting swarmed by zombies.

If doom saved a piece of an earth he could easily save the people on that piece as well, he has the power of a god. Also Dr. Strange said to everyone on the life raft that Doom did save some people from their universe 616. Some small part of Wacanda and a little over half of New York. That is about 4 million survivors from 616. Maybe 3 million from the Ultimates universe in their half of New York.
