Probably Sabretooth. On one hand he might seem like an average villain bordering on anti-hero, but he's done some very groos and weird things, and in the MU there's like an 85% chance of coming across him. Not a good time.
There's a character from the Warren Ellis/Leo Manco run on Hellstorm called the Bailiff of Madness. It is essentially Hell's concierge (with all the warped functions you'd think would apply.) Its body seems to consist of thousands of spider-like creatures, but it's sartorially inclined to wear a top hat and Edwardian suit, which makes it all the creepier. And then there's the politeness...

I am not sure about the creepiest, but think Mr. Hyde is a scary guy. He seemed really unhinged in the Under Siege storyline.
In my mind, Blackheart. I look at him and see evil made flesh.
Also, Carnage (less so recently), Shadow King, and the Brood...
