Kamala and Miles learned each other's secret identities in one of Ms Marvel's solo issues, I think. And at the start of ANAD Avengers, Nova introduced himself to Kamala but she didn't believe his name was Sam for some reason. Later, Sam Wilson calls him Sam, revealing to Kamala, and the whole team, his identity.

That still leaves the question of when Sam learned Miles's name.

With the reveal of Spirits of Vengeance's creative team, it already seems to be debunked.

Besides, the rumor seemed tailor made to cater to those who didn't and don't like change, implying that the company oh-so "secretly" hates it, too. That made it suspect in and of itself.

All the the previous "Ultimates" lines were minis/maxis in the Ultimate Uni. Ewing's 'Ultimates' were following the seasonal pattern so basically, yes.
