Three times in an anime series the same woman pops up for no apparent reason. The character design is average but recognizable, so I doubt it's a celebrity cameo. Maybe she's some kind of continuity, or, most likely, an inside joke by the animators.

Most likely, they just reused a stock shot or just repurposed the character design. Happens all of the time in animation. Saves time, especially if a deadline is looming.

Could be. If anyone has Jigoku Shoujo, the character appears in episodes 18 and 23,season 1, season 2, 15.

Forgot to describe said character: wears glasses, hair pulled back in a bun, ear length bangs side of face, thirtysomething.
