Bigger clarification: Snake beat the 2nd man to earn the name Big Boss (Venom Snake) and then defeated the 1st Big Boss later on his career before going on to defeat Liquid Snake. That and a good dozen of elite soldiers. Oh and a few superweapons.

He was able to do everything BB did and no one suspected he was a phony. Damn impressive for a medic body double.

yep. it was done to fool the world that he was BB and doing this and that and other things. and he did such a good job that no one knew, save for ocelot, who was in on it all, and kaz, who told ocelot the truth.

This. Venom Snake was skilled enough to fool the world that he's the real deal. This is enough from Big Boss himself to declare Venom the man worthy of the title Big Boss.

After that, he did manage to not only endure a brutal fistfight with his opponent, he came out on top.

All this, and he was physically (not chronologically) in at least his mid to late 60s...
