What if Cap didn’t die after Civil War and actually stood trial?
He would make a very passionate speech against the evils of the SHRA and sway so many people to his side, because Cap has such strong conviction in what he believes and stands for that it would be almost impossible to convincingly portray him as the real bad guy, especially if or when some of the worse human rights violations committed in the name of enforcing the SHRA become known to the public. That was really why he had to die; the SHRA would completely fall apart in the eyes of the general public if he were allowed to speak openly and frankly of the wrongs committed in the process of its enforcement.
As for what I want to see, What If . . . Spider-Man Became Unlimited? Basically, think Batman, Incorporated, but with Spider-Man and all the other spider-powered or -themed heroes who've shown up over the years uniting across the globe to spin a web of great power that also came with great responsibility. Spider-Man Unlimited!

What If all mutants were labelled like terrorists the world over.
I am a Marvel fan preferably cosmic storylines, especially Thanos or Dark Phoenix related, when both the Avengers and the X-Men are involved count me in, loved the original Uncanny Avengers series.
Not a fan of any of the new characters.

What if Marvel had their own Crime Syndicate universe?
That's already canon, to a certain degree. Given how human governments across Marvel Earth have sanctioned some of the most repressive acts and policies to curtail "mutant violence" or "mutant terrorism," which in practice have amounted to criminalizing mutants for existing, it's pretty much a fait accompli in the Marvel Universe that if you're a mutant, you're automatically a threat to humanity and law enforcement and/or other authorities are thus well within their rights to deal with you however they deem necessary.
That said, my idea for a What If . . . What if the Incursions were really Convergences that merged instead of destroying alternate realities, consolidating the Marvel Multiverse into a more unified whole?

What if Spider-HAM was Ven-HAM?
I think this could be interesting and go in several directions. Bucky likely wouldn't be taking on the mantle yet (or at all). You have Sharon Carter's pregnancy, which Steve would be there for. I assume she wasn't brainwashed for him to not be shot. Tony's taken up more with SHIELD, and I wonder how Steve still being alive would affect that.
What if the Eternals killed off all other species of humans on the Earth?
Imagine Marvel putting out a limited series that introduced an Earth where 99.9% of the planet was a cosmic powered Eternal. Then maybe have a group like the Avengers go there to try and save the last 0.1% of normal humans from becoming extinct. And, of course, no jobbing allowed.
What if Mar-Vell had never contracted cancer, and came into possession of Quasar's Quantum Bands?
Funny enough, Chris Yost's New Warriors had a somewhat similar plot, with the High Evolutionary, at the behest of who he thought was the Eternals, trying to purge the Earth of all other variants of humanity that deviated from the baseline --- clones, mutants, Atlanteans, Inhumans, extraterrestrial hybrids, supernaturally touched, etc.
