Based off of Polaris being mentioned...and not knowing that Wolverine had died and there was a whole set of books and a couple of years dedicated to "finding him" across the MU...I could hazard a guess. (I was not following the arc and even I knew that)

That aside, many of the responses do raise interesting questions for me like what counts as death (does time travel erasing them from existence count?), and how often has Marvel done fake-outs for cliffhangers.
Starting to remember, too, all the complaints about Rosenberg killing off characters leading up to HoX/PoX before anyone knew its plans.

I agree with salarta, "death" counts should be where a funeral has taken place or where there are ashes or a body and people were mourning, not a mass event where alternate universes collided and wiped out everyone or stuff like that.
There have been so many universal blink outs over the years, they have all died to one extent or another.
